1. Referral

For spine-related injections, patients will require a referral from a physician or nurse practitioner. In addition to the referral, patients are required to complete a questionnaire in order to Book a Spine Injection Appointment .

The referral and completed questionnaire must be sent by email to info@canmagnetic.com for processing. Alternatively, you may also upload your completed forms directly to us here.

Patients will require a referral for all treatments performed and complete a questionnaire.

2. Scheduling

Once we are in receipt of both the referral and completed questionnaire, one of our specialists will contact the patient directly for a short, virtual consultation. After the consultation, the Spine Injection Appointment will be scheduled.

3. Consult/Treatment

At your appointment, a specialist will review your condition and, if appropriate, perform the recommended treatment.

All treatments will take place at 2225 West Broadway, Vancouver, B.C. V6K 2E4.

4. Results

Reports of the treatment performed will be delivered to the referrer and to the patient if requested by the patient.

Payments will be collected at the clinic at the time of treatment. Forms of payment are Visa / Mastercard / Debit.
